Activity Description:
In the high-stakes world of pharmacy, understanding how to navigate risk is essential. This interactive session explores the principles of Just Culture and the critical role of error reporting in promoting patient safety and continuous improvement. Participants will gain insight into the distinction between latent and active failures, identify contributing factors to mechanical medication errors, and understand how professional liability insurance protects and supports pharmacy professionals in their practice. Designed for both pharmacists and technicians, this session offers practical strategies for fostering a culture of safety while managing professional responsibilities.

At the conclusion of this program, the participating pharmacist or technician will be able to:
1. Explain how Just Culture and error reporting help to prevent errors through education and system changes.
2. Describe the difference between latent failures and active failures.
3. Identify common types of medication errors considered mechanical errors and the contributing factors to those errors.
4. Describe how professional liability insurance plays a role in your career and practice.


Texas Pharmacy Association (154) is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. Continuing education will be provided to participants who completed all course requirements.
